HOW IT WORKS

• Deductibles – If you have not met your insurance plan deductible, we recommend you do not use insurance to purchase your iWALK crutch.
• You are purchasing the iWALK crutch through Medequip, Inc.. Medequip is not affiliated with iWALKFree, Inc.
• Every insurance plan is different and Medequip cannot guarantee your insurance plan's coverage or reimbursement policies.
• In the case that your insurance will not cover or reimburse for the purchase, you are responsible for the standard online purchase price of $159 + sales tax, if applicable.
• When you submit this completed form, your order will start processing immediately, and the order cannot be cancelled or returned. Typical delivery is 1-4 business days. (Business days are M-F excluding holidays)
• If you prefer to purchase the crutch WITHOUT insurance, you can do so by selecting the “Buy iWALK without Insurance” button at the bottom of each page.

• You will provide information needed to process your claim. This information will be transmitted to Medequip, whom will process your insurance claim.
• Your iWALK3.0 crutch order will be processed immediately for fast shipping.
• A representative from Medequip will review your information and contact your insurance. Medequip will then call you within 1 week to discuss the outcome or next step.
